A glitch has occured

I was in a stronghold, and got many items. I said, "brb" and then typed /home. When I came back, I was dead, so I typed /home while at spawn. Unfortunatly, I started getting hurt and then had to goto spawn. When I went home, it only showed black, and was just hurting me. Previous attempts to go home were successful though. May you please tell me what happened?

Possibilities:
1)Some idiot griefer covered your /home coordinates with some blocks of some sort.

2)You were experiencing a strange chunk glitch that might get fixed after a restart or reopening minecraft

3)Somehow your /home was tampered and moved into the void?

4)someone decided to WE out your house area.

Also, what does it say when you die? Suffocation? Suicide? Falling?

Suffocation and falling once. It was really annoying...

That happened to me once you need to dig under your house because thats where you are spawning

Everytime i /home i have to relog or i fall into the void and die, pretty normal thing.

./home then /jumpto.

Build a block under your feet when you set your home point, otherwise there is a chance you will spawn in the ground and suffocate.

This time I fell to my Derp! I lost maybe 500 gil worth of stuff...

Teleport, log out, log back into server.

That's what I did do, and then I just fell through an invisible hole... I'm so freaking annoyed...

Maybe someone put a hidden trap door under your home point for lulz?
